An “incident” is defined as a consequence to a person or property.
Incident reporting is required by law

Promptly report all incidents, accidents, and hazardous conditions to the Technical Standards and Safety Authority (TSSA)
Amusement Device owner and operators must also:
Stay current with all licensing requirements for amusement devices
Report any owner information changes within 10 days.
Ensure devices are operated in accordance with all regulations and manufacturer’s specifications
Hire only qualified, certified mechanics to perform maintenance work or make alterations to equipment.
